چکیده
The American Society for Gastrointestinal Endoscopy (ASGE) Technology Committee provides reviews of existing, new, or emerging endoscopic technologies that have an impact on the practice of GI endoscopy. Evidence-based methodology is used by performing a MEDLINE literature search to identify pertinent clinical studies on the topic as well as a U.S. Food and Drug Administration Center for Devices and Radiological Health (MAUDE) database search to identify the reported complications of a given technology. Both are supplemented by accessing the related articles feature of PubMed and by scrutinizing pertinent references cited by the identified studies. Controlled clinical trials are emphasized, but in many cases, data from randomized, controlled trials are lacking. In such cases, large case series, preliminary clinical studies, and expert opinions are used. Technical data are gathered from traditional and Web-based publications, proprietary publications, and informal communications with pertinent vendors. Technology Status Evaluation Reports are drafted by 1 or 2 members of the ASGE Technology Committee, reviewed and edited by the Committee as a whole, and approved by the Governing Board of the ASGE. When financial guidance is indicated, the most recent coding data and list prices at the time of publication are provided. For this review, the MEDLINE database was searched through July 2015 for relevant articles by using the key words “per-oral endoscopic myotomy,” “endoscopic myotomy,” “POEM,” “achalasia,” “nutcracker esophagus,” “jackhammer esophagus,” “diffuse esophageal spasm,” and “laparasocopic Heller myotomy.” Technology Status Evaluation Reports are scientific reviews provided solely for educational and informational purposes. Technology Status Evaluation Reports are not rules and should not be construed as establishing a legal standard of care or as encouraging, advocating, requiring,

The Choice of Myotomy in Achalasia Cardia: Heller's or Per-Oral Endoscopic Myotomy
Achalasia cardia (AC) is a neurodegenerative disease which leads to loss of inhibitory myenteric neurons. This implies that, with the currently available treatment options, we offer palliation but not cure to these patients. Pneumatic balloon dilatation (PBD) has been the only time‐tested endoscopic modality for the management of AC until recently. However, the requirement of repeated dilatatio...
متن کاملFirst Chinese Human Gastric Peroral Endoscopic Myotomy for Diabetic Gastroparesis
Diabetic gastroparesis is a chronic functional syndrome caused by diabetes mellitus. Diagnosis is based on symptoms and scintigraphy. The therapeutic options are still limited. A publication has recently described the effectiveness of gastric per oral endoscopic myotomy. We herein present a case of diabetic gastroparesis treated by gastric peroral endoscopic myotomy which extended the submucosa...
متن کاملEndoscopic Management of an Intramural Sinus Leak After Per- Oral Endoscopic Myotomy
Per-oral endoscopic myotomy (POEM) was developed less than a decade ago for the treatment of achalasia. Its minimally invasive approach and the favorable short-term outcome have led to rapid adoption of the technique throughout the world. As with any new technique, there will be adverse events, and it is important that effective treatments for these adverse events be discussed. We present a cas...
متن کامل[Per-oral endoscopic myotomy in achalasia].
The treatment of patients with achalasia is complex due to a considerably high recurrence rate and risk of treatment-related complications. The per-oral endoscopic myotomy (POEM) is a new endoscopic technique that combines the benefits of a minimally invasive endoscopic procedure with the efficacy of a surgical myotomy. Implementation of the POEM technique may lead to fewer complications, a low...
متن کاملSame site submucosal tunneling for a repeat per oral endoscopic myotomy: A safe and feasible option
Per oral endoscopic myotomy (POEM) is a novel endoscopic procedure for achalasia treatment. Due to its novelty and high success rates, a repeat procedure is usually not warranted, making the feasibility and safety of such approach unknown. We report the first case of a successful repeat POEM done at the same site of a previously uncompleted POEM. An 84-year-old female with type 2 achalasia pres...
متن کاملThe last innovation in achalasia treatment; per-oral endoscopic myotomy.
BACKGROUND/AIMS Per-oral endoscopic myotomy (POEM) is a minimally invasive endoscopic treatment option for patients with achalasia and has been performed since 2010. It is less invasive than Heller myotomy and its use is spreading rapidly worldwide. We present our results of POEM that, to the best of our knowledge, are the first cases in Turkey. MATERIALS AND METHODS We enrolled patients betw...
